I have someone in my family tree whose parents are unknown but whose
sister's name I do know.
I can't find any way to add the sister as a sibling except as a child of
named parents.
Do I have to invent fictional parents to add this sibling to the tree?

I just went to family view, added a new family, added a test person to
the family and saved it without adding any parents. It worked just
fine!

Brian, this is very common if you do much census work. Doing a one
name study, I have many of these. To start with you do know, in most
cases, the last name of the father. I use this marking for unknown
people. "(?)" for unknown given names and "(Winfrey)" for last names
of females where I don't know the maiden name. So, in your case I
would have (?) Winfrey for the father and (?) (Winfrey) for the
mother.

I stopped using "?" a while ago. You don't need it. Also, I just

I think that that is only possible in upcoming 3.0 If it by accident is
possible in 2.2.x then it would be a bug, as some/most code in 2.2.x expects
there to be parents in a family
